It means your skull is so thick that information cannot be received by your brain, implying that you lack the ability to learn and that the only information that does pass through to you has to be 'Hammered' into you for it to stick. the overall implication being that :-
a)you don't listen
b)you're stupid
c)you'll never learn anything (easily)
d)you're ignorant
e)in it's extremest form- you are mentally challenged!
It is in all circumstances an insult and a horrible thing to say to someone. If anyone has said it to you put them onto me and I'll give them what for!
Take Care
The inquisitive mind questioner.

What does "thick head" WHERE mean? You gave no example or citation! Generally, describing someone as being thick-headed means that you think they are stupid or stubborn. Which of those the speaker means should be obvious from the context or situation.

It means that person don't listen. When I was little, my big brother (the oldest) would always say that term to me because sometimes I would disobey my parents and get in trouble. Now I stay away from him as possible.
