How do you say "In Loving Memory of" in Italian? Please help.

I think I need to explain myself further. I want to put an "in loving memory" tattoo on my lowerback. How would I write "In Loving Memory of Cody"?

"In loving memory of" literally is:
Nell'amato ricordo di (Cody)
Anyway I'd prefer this one:
Nel caro ricordo di (Cody)
"amato" is "beloved" but "caro" (dear) in this case is more appropriate.

it's actually:" Le piu' sincere e sentite condoglianze".
But if u want to translate that literally it'd be "Alla cara memoria di...."

"Le piu profonde e sentite condoghieuze" which means "deep condolences."
"In effittosoi memorie di __________" means the same as "In loving memory of ________"
