Only include "Sir" if he's been knighted and given that title. Otherwise, it should be "Dear Professor Smith."
If he has been knighted and given that title, then it would be "Dear Professor Sir John Smith", but "Dear Professor Smith" would also be acceptable.

If you already know the man, it's "Dear Sir John", however, if you are only a 'nodding acquaintance' then it's "Dear Professor Smith" .
