The other answers are right to an extent. The easiest and most accurate way is to call your employer and get a "reissued W2".

However, if you worked for someone you can no longer contact, or if the company doesn't have a copy for some reason, then you need to get a "transcript" from the IRS. This is free, and will have the information from your W2, though not in the same format. An IRS transcript is the same as a W2 for the purposes of filing the return.

To get a transcript, you'll need to file a Form 4506 -T. I've attached the IRS link so you should just be able to print it. (When I tried the link I got a message that said some things may have changed on the form. This is because it's been linked on the Yahoo site, not because I changed something on the form. But in order to be safe you might want to find the form straight from IRS.gov). Then you'll need to either call the IRS (1-800-TAX-1040) or look on the website to see which location you'll send it to. (THis differs according to where you live).

RE:
How can a lost W-2 be replaced?

i used to be a payroll manager so
my suggestions are:
call your employer, because they SHOULD have a copy of the w-2. if they don't or if you can't find the company
most w-2 copies are sent to the government dispite what you do, so you could call llike a ss office in your area and ask them how you can get a duplicate w-2

these things are kinda different for each state, did you try a google search for duplicate w-2 and the name of the state? hope this helps!
