From my experience, in situations I have seen, shouting usually occurs from the failure to communicate. On occasion I have seen fists fly after shouting all because both parties try to express their opinions without even hearing what the other person has said. That's when it becomes a stand off on who's more dominant than who and whomever feels like they are loosing first usually gives the first punch just to make the other person hurt more. Usually this type of conflict doesn't care what's happening around them as they are too fixated on each other.
I have seen attention seekers who start fights just to prove their worth by name calling and belittling based on their personal emotions of another person and those type of people are just out for a reputation, my thoughts on that is, they just prove how dis functional they really are.
I have just noted what I have come across BUT I cant speak for other situations.

They don't always do that. I was cussed out the other day by some idiot waiting for a ride. He didn't raise his voice but I looked around and thought he was talking to someone else. I couldn't figure out what he was cussing about but he said too much and I walked over to him and punched him in the jaw. He did start raising his voice then. He was just an idiot but I was an idiot to have punched him. I had no anger but instinctively just did what I did with no thought, reserve, anger, adrenalin rush, etc. I just did what I did and had no apparent connection to my actions until I thought about it later. I can't say I was out of control but apparently was because I abandoned all emotions, thoughts, anger, feelings, or whatever else I can think of. As if my body just did what it did seperate from my mind. I have to deal with this and wonder why I went into this mode I have never experienced before. He was wrong and probably mentally disturbed or on drugs when he cussed me out. At least he had a reason or so he thought. I could not reason with this person nor interacted with him in any way to have been cussed at. I risked my freedom by punching this person. I learned a great deal from this and learned something about myself that I am still trying to figure out. Usually a thought or emotion goes before actions and I lacked anything similar. Sort of a reptile response that is very primitive and I wonder where it comes from.

Anger builds momentum, shouting is a stage. It's also a sign. When the shouting starts it's time for everyone to go away and let things cool.

No...I think whatever was said before hand was just getting the agression out before the first blow. Alot of people like to talk smack to see how the other person reacts before starting the actual fist fight. Alot of talk before the fight sometimes fixes itself before getting into the physical routine of things.

It goes back to the apes. They yell and scream at each other when they are aggressive. Some people still drag their knuckles when they walk, I guess.
