come up with a melody .how you want to sing it. then come up with a good sounding chord progression that works with the melody also try to come up with a rhythm by clapping, snapping or using empty 2liter bottles with the caps on as drumsticks hitting them together .also think of the bassline. get more musicians if you can.

depends on how skilled your player is. I've been playing for about three years, and don't practice as much as i should, but the way i would do it, is have the two of you sit down and come up with a tune, and find chords that fit the sound you want. alot of times, when you use a couple of chords, you have a group that goes along with that.

I assume if you write lyrics you also write a tune to go along with that? It really depends on what type of music you play, what sort of progressions you like and if you like harmonics and stuff like that. The easiest answer I can give you is this... sing a line of the song and hit notes on the piano. Whichever note sounds best at any given time should be the one your rhythm guitarist plays.

Well, every song is sung in a certain key, as the song plays you change the pitch and key to a higher or lower note. If your guitarist is skilled he could easily find the key in which your voice is in and find it on his guitar. Then follow along with you.
