I would ask her to assess her performance so that you can get a handle on how far she is from your observation. Then I would explain what you see happening and the impact of her behavior. Develop a plan to get her to where she thinks she should be.

If you intend to give an employee a bad review, make sure she is deserving of the bad review regardless of who she thinks she is. Nonetheless, you still need to point out what her strengths are first and then break the bad news.
I hope that helps.

I had to do the same thing about a couple months ago. The best advice is not to let your personally feeling get in the way, be honest, but most imnportant do it with class. Let here know what needs to be changed and why it needs to be changed.

Just my opinion, but you have not done your job, if she thinks she is doing hers (well)... and infact, is not.
Every employee should be given the training and timely followup, as well as feedback, and/or constructive criticism, to succeed, or fail.... no employee should be "surprised" with any "review" that He (or She) receives.

Well, have you talked to her before about problems when they have arisen? If you have, then it's fine to give her a bad review. If you've been saving all this stuff up to spring on her, that's not fair IMO.
