She will have to go to the board of registry or City Hall in her town get duplicate forms and then have them filled out by whoever performed the ceremony and register the paper work.

She needs to have another reissued as soon as she can....I had the same problem , and she will need it if she has to deal with the government in anyway to prove change of name. I needed it when I went to renew my drivers license...if it had been filed she should have no problem getting a new one...Hope this helped

Go to the court house and have one issued. Normally they have the documents in file. They will issue you another one and the process doesn't take very long.
