The only way you can get close to actually having blue balls is if your name is "Papa Smurf". If the guy is very aroused and doesn't ejaculate, he experiences discomfort in his balls. It doesn't necessarily feel painful but these is strong urge to go masturbate. He needs the release. The balls do NOT actually turn a bluish color.
