When getting critical surgery in the USA, does one have to pay before or after the surgery is conducted?

What if one cannot afford it? What are some financing options?

In the USA you can always apply for medicaid. It is a state provided health insurance and is for low income people. There is always a chance that you may not qualify for strait medicaid and then they have a program called medicaid spendown. After you pay so much then the state will pick up the rest. If you are not low income you can always make a payment plan.

If it's critical surgery, you shouldn't have to pre-pay. There's truly no way to know how much the surgery is actually going to cost with everything involved. If you have insurance, you might be asked to pay your copay before the surgery, but if it's an emergency, they will probably just bill you for this too.

If you are admitted to the emergency room and your life is in danger, you will be taken care of...no matter what your financial situation is.

I don't think you can just schedule a surgery though without the hospital asking for some medical insurance or how you plan on paying for it.
