Need to go to the Dr now. It can be a problem called Torsion.That's when the testicle turn or twists inside the Scrotum.The blood flow gets cut off and tissue starts to die,the tissue being the testicles.My son had this and doesn't remember an injury that could of caused it.He woke up hurting and by the end of the day he was in surgery.Look for a blue dot near the top of the scrotum where the testicle usually sits.The dot helps indicate if the testicle has turned.the dot is actually a bruise(blood)that shows that torsion has occurred,there is tissue that holds the testicle upright and somewhat centered.If its not there still go to the Dr.Don't mess around with this issue.ALWAYS better safe than sorry.

orchitis - inflamation or swelling of the testes. He needs to a see a doctor immediately to have this assessed. Leave it too long, and they may need to amputate. See the link below for more info.

could be almost anything from an injury to a hernia to who knows what - he needs to see his doctor to really find out since without examining them no one can really say what is causing his swelling.
