The all time classic psychological thriller is The Changeling with George C Scott. Every critic horror rating I have ever seen has it in the top 3.

I found American Psycho very, very scary ... it isn't entirely lacking in blood and gore but the atmosphere is creepy (although I have to say that not everybody agrees with me).
Sleeping With The Enemy ... excellent. Also Klute; Don't Look Now; and on quite a different scale the Ian McKellern version of Richard III.
Enjoy your shivering ...

Stir of Echoes, Play Misty For Me (old Clint Eastwood movie) very good though, Cape Fear, Fatal Attraction, What Lies Beneath. These are all good psychological thrillers.

watch saw the plot is Two men wake up at opposite sides of a dirty, disused bathroom, chained by their ankles to pipes. Between them lies a dead man loosely clutching a hand-held tape player and a handgun. Each finds a tape the perfect fit for the player in their back pocket. They play the tapes. One is threatened, the other isn't. But they have a task: One must kill the other by 6:00, or his wife and daughter will die

When Darkness Falls is creepy with no blood and gore....it makes you afraid of the tooth fairy though.
Also, I think all the Psychos are pretty scary. Also try Silent Hill

Happy Birthday To Me (1980), that's if you could find it.
i know you said you hate blood and gore (this one has a lot of it), but this one was scary for me when i first say it.
next time you are eating a cake, remember there was one that once had a dead guy's head in it.

Maybe a classic like "The Rear Window" by Hitchcock or something like "The Others" with Nicole Kidman
