This tendency appears to be innately programmed into our brains. Here are a couple of interesting articles on the subject.

I'm not quite sure at what time this is determined. I just watch which hand a baby uses to grab things - then I know if they are left or right handed. One of my grand daughters grabbed either hand, eats with either and writes with either, however, she does use the right more than the left.

This is MOSTLY attributalbe to genetics and so is determined at conception, but there are other (environmental) factors, too, that contribute. Handedness can be seen very early, but it is generally clear by school age.
Both left handedness and handedness are covered in Wikipedia and the articles are good and thought-provoking. (For example, could right handedness be an evolutionary advantage and are we de-selecting left-handers (over time)?)

I'm left handed and both my children are right handed. As soon as they start to pick things up you know by which hand they prefere. So very early on.

Before they're born. I have an ultrasound of my daughter with her lft hand in her mouth. Possibly it doesn't mean anything, but she is left handed.

My two nieces demo'd left handedness when first trying to feed themselves. I think that you are predisposed to being left or right handed.

It's not until babies start playing with toys that you know which one they're going to use, but their brain new before birth. You can retrain it with enough practice if you wanted to, but why bother. Let them use use whatever hand feels more natural to them.
