Only in awareness, which has no cause, no centre, can this truly be realised.

The main cause of human suffering? That's a pretty braod statement and would have to say no. I believe generally in the Western world more specifically here in America this is probably more true due to our society and the importance it places on things. The message of our culture is one of seduction and self pleasure = happiness.
If one was born into a war torn state, with famine and plagues one could suffer with out attachment to Form, unless you would consider the desire for basic human needs and lack of it attachment. This person may find it unfathamable to be attached to form, when the possibility of basic needs are inconcievable.

My answer is no
Main cause of human suffeing cannot be explained so simply because people suffer due to more immediate things such as greed, ignorance and need for power and control. A person starving in Sudan is not afflicted with identification with form and neither is the person causing the starvation.

A little more than that.... not just the identification... the inability to maintain or regain the experience that this identification is not absolute, irrevocable, permanent.
If we realise that it is a mere identification, we are free from the attachment, and now we can enjoy this identification IRRESPECTIVE of pain or pleasure, and can drop it AT WILL .
