usually just grows back unless the root was severly damaged too. i smashed my thumbnail with a hammer a few years ago, turned bluish purple then fell off and grew back after a few weeks. looks normal, its not a freaky mutant nail or anything.

Yes. Normally a replacement will grow underneath the old nail. Finger and toenails are dead tissue, like hair, and when one falls off that's because it has broken. It continues to grow behind the damaged part.

The new nail will be somewhat soft at first but will harden and after a few weeks your finger or toe will be good as new.

Yes. My toenail fell off after volleyball season one year. It just kind of hung there by a thread for a few days (because I too, was too afraid to pull it off in case it hurt.) However, finally I just pulled it off, and it wasn't painful at all. So don't be scared. It really can be quite fun to show people your disgusting nail-less toe (or finger!)

I slammed my middle finger in the door and had to get it drilled. It eventually fell off and a new one grew in, but the new one looks slightly different from the other fingernails. I think I am the only one who notices that it looks slightly different. I also notice that it can crack easier than the other nails.

It happens all the time, it will grow back. As to why they fall off sometimes, I don't know why, they just do sometimes.
