Fork on the left, knife in the middle, and spoon on the outside beside the knife, with the knife facing the spoon? Or, fork on the right, spoon on the inside left, and knife in the middle, again facing the spoon.

Plate is in the middle.
Fork is on the left of plate.
Knife is on the right of plate, next to the plate facing the plate.
Spoon is on the right of the plate, and right of the knife.

Water glass is just above the knife.

Napkin if not in a napkin ring, in the center of the plate, is laid next to the fork on the left side of the plate.

Salad bowl or dish is just above the fork/or this could be your butter/bread dish, with small butter knife laid across the top of the small butter/bread dish.

I was always told:
Fork on the left of the plate
Knife on the right of the plate with the blade facing the plate
Spoon to the right of the knife.

Fork on left, plate(s), knife (toward you) because if you place it facing other guest it is taboo), and then the spoon.
