Sleep today, and tomorrow go to the school`s phych and declare deep emotional stress. Cry and explain everything, including the exam. If you are trully stressed it will be seen tomorrow and she might write you a not.

Get back together with him for this one week. Your mind will be at ease and you can always break up with him again later. You will kill two birds with one stone this way. You will ace the test and you can get out of the relationship again if you so desire.
If this is not possible, then everytime you think of your ex-boyfriend, replace the image of him with something you really can't stand. If you hate him then this should be relatively easy. If you still love him, then you should try to replace thinking of him with something else that you love, perhaps even more --- such as a mother or father or someone else that you admire.

I'm not sure if this'll work, but you could try filling your head with something else, like classical music. Wagner, for example. Or something alternative, like Enya. Hope that works.
