This depends on what state you live in if you are in the United States. This is extremely important since the law differs from state to state and if you do it wrong you are probably SOL.

In New York State its called a Health Care Proxy. The NY Attorney General provides information on its website:

If you have a Doctor, he or she should have an original and/or complete the form with you since they are the ones who will need it if you become incapacitated!!!

It's called a Living Will. It's the best thing a person could ever do for himself. You can get one on line or at the hospital.
