Try stapling notes to her forehead. It will get much worse before it gets better, trust me.

you need to sit down every morning and make her write a list of to-dos that she needs to do that day. This is a very good skill and will need it when she gets into highschool. Do this everyday, no matter what, because you will want this to become habitual. Trust me. If I didn't have a list I don't think I would have made it through AP classes.

post post it notes around to remember. Have her tell you then you tell her before she goes to school or whenever she is going some where have her write it down. don;t let her watch as much t v or play as much video games.

why should she remember anything when you do the reminding for her? let it be her problem and when she has to pay the consequences she will do better.
