You used the word committed though. Most people are committed either against their will because they are unable to make a cognitive decision or because they have done something which endangers their lives such as a suicide attempt.
In some states, a member of your family can petition the court for a commitment if they are concerned about your being a danger to yourself. Depending on the policy of that state, you could be taken for a 72 hour (or whatever is the legal limit in that state) assessment at a hospital. After that, a decision could be made about whether you need additional hospitalization or some other form of treatment.
People who admit themselves are in most states also able to dismiss themselves. If they are court committed, they usually do not have that option.
As far as insurance coverage goes, I am not even going there...mental health insurance coverage is normally not equitable with physical health coverage. This is called parity and you will find that many states and the federal government itself are grappling with this issue right now and have been for sometime.

If a person is a danger to himself or others, they can go to any emergency room. If someone has insurance, mental health institutions will be more than happy to treat you till the ins. runs out, usually about 30 days. Otherwise, a state run institution will take someone in who needs help.
