Every six weeks or so I get a "blemish" at the corner of my mouth or right where my lips meet the skin. They start off as small painful dots, but I can't tell if they're zits or cold sores. If I put anti-acne medication on it overnight, it will form a big white head by morning. I guess it dries out and "pulls" to the surface. Gross.

Should I treat these as acne or as coldsores?

And if it's acne, what can I do to stop getting them?

It sounds like a zit, I would treat it and prevention as any acne. Wash, medicate, exfoilate, etc. A cold sore wouldn't get a "white head" on it as much as blister and break open. It would look like an "open sore" since that what a cold sore is.

It's definitely acne... not a cold sore.

As Peter P pointed out, a cold sore never forms pus (a whitehead). Plus, a cold sore is tingly and really hurts and it takes about 7 days to heal.

Best way to deal with it is to keep the area clean and apply a small amount of your acne medication everynight, even when you don't have a blemish.

If its coming back in the same place all the time, I would guess coldsores which is actually a herpes virus. It threw me off alittle when you said that it forms a white head after you put acne medicine on it at night. I guess your best bet would be to see the doctor and let them decide.

If it is reoccurs every six weeks in the same spont its probably a cold sore. And I'd treat it as a coldsore.

it sounds like acne, to avoid getting them in the same area every time try, and it might sound funny, but clean your cell phone often. A cell phone carries many germs on it, especilly the mouth area on it. I'm sure you probably share your phone too. So try that, it helped me wit the same problem.

It sounds like a cold sore(Herpes Simplex1). Try Abreva on it. but you must use it when you first notice the dot. Or you could ask a doctor for a presctiption for Acyclovir.
