write down your goal, set a target weight and a date, make sure you follow-through, dont' get frustrated if you're not on pace, make a bet with someone, think long term, think about getting a head start now before the new year, every week should be an accomplishment, think about obtaining a better quality of life, you did it once you can do it again, don't think about the past keep pushing forward, where will you be 2 years from now?

You probably lost weight 2 1/2 years ago through a deprivation diet and not by eating healthy.
Gym alone won't help you. Actually it might give you the false sense of security that "I worked out tonight so I can now eat whatever I want". That is why people who do only gym workout to lose weight are often surprised that it doesn't work.
This is because you spend far more time out of the gym than in it. It is not humanly possible to burn more through the gym than you eat if you are not eating properly.
You need to learn about proper nutrition and why deprivation diets all eventually fail (you are a typical example of why, not trying to be mean or rude, but you are not alone).
You will find that what you likely did was destroy your body's muscle stores. That is why you gained the weight right back AND put on more because this time around you were consuming the same quantity of food as before but with less muscle to metabolize it quicker.
You should start by reading up on nutrition. So far I think the best book (no-nonsense and it is not trying to sell a particular "diet") is Eat Drink and be Healthy, The Harvard Medical School Guide to Healthy Eating. Begin by eating healthy. Cut out the High Fructose Corn Syrup (read labels, it will more than double your chances of getting diabetes), and Trans fat (simply put it is a substance put in food that lets it "keep" longer essentially an artificial fat that "re-hardens" in your body after it is broken down).
Then concentrate on excercise. Excercise is at most maybe 20% of the game. It is a supplement to help you lose weight and get to that final "fit" body you want not the answer by itself.
EDIT: Your other mistake was to set a target weight. Unless you have a lot of UPS packages to mail out. Throw out that scale. Weight is the most INACCURATE way to measure, BMI is not that much better (It gives funny results for physically active people). Go to your local gym and have them test what your body fat % is. This is the most accurate way to tell. Then every month have them measure you again so you can see your progress. Or you can buy a home kit (calipers) and learn to use them properly, as there is more potential for mistakes when you do it yourself you should still periodically have it done by a trainer. Setting a target weight is setting yourself up for failure, just develop and stick to a healthy eating plan and workout plan and watch the changes. You will find the most drastic changes in the first few weeks then it will taper down but you should still see some improvement.

Also try starting slow. Some people try to rush into things and don't go step by step. First start with you diet and do light exercising (walking, light jogging, walking the dog).
Diet is important. I always trick people into a diet by saying just do it for two weeks. Just give me two weeks and if they can you end up feeling so good you really don't want to go back.
Also what are you doing to exercise? If your doing something you don't like chances are its not going to be on the top of the to-do list. Try something thats fun to you. Try a kick boxing class or some other class. Also a workout buddy never hurts.

One of the reasons you're probably so sluggish is because of the lack of exercise. I find it helps to have a buddy to exercise with. You might also try different exercise techniques, jogging around a track gets boring and I don't blame you for not jumping with excitement for that. But try kick boxing or tae bo. The fast-paced cardio is a great work out and learning kicks and jabs is a lot of fun! Try not to think of exercise as a chore, but a new hobby to learn skills. Good luck!
