using a false name is only an offence if you are commiting a fraudelent act. If you dont want to know the doctor to know your name providing no other criminal act is being hidden because of that you are not breaking the law.

In terms of perscriptions I found out recently if you go to casualty they give you free perscriptions. They just gave me some medicine and never asked me for money.

Yes. Every medical event in your life must be documented, as this could save your life in the future.

Besides, if you give a false name, the doctor will know, as they check the records of every patient before you see them. If they can't find your name, and you can't supply a valid reason, they will not see you.

yes-it's a criminal offence-it's called obtaining goods by deception, even though the doctor might not refuse to serve you if you had a serious condition.

Yes of course it is..say they picked up the wrong notes and treated you for something different or you have something catching, they will never find you.

Yep. What if you suddenly fall dead or something? I think they need to check your medical records if that(or somethin worse) happens.

well the doctor will have your medical records so they will know your real name...why would you want to do this anyway.
