There are a lot of factors which may be facilitating your "thinking about a million things at once". Number one, they cannot test for a dysfunction in the body to determine ADD, they can only listen to what you are saying about how you are feeling. If it fits into the category of a medically created dis-order, they will suggest drugs.
Here is another suggestion. It is a well known fact that people who experienced trauma before the age of 8 years old, where the adrenals kicked in, (the fight or flight syndrome) and that trauma has never been resolved, the sub conscious triggers will fire and the fragmented thinking happens. People who are hyper, hyper sensitive, have what is called a continuous adrenalin drip going on 24/7. There could be 1000's of triggers to the old file. A feeling, a smell, a sound, a thought. All of them have the potential of kicking in the drip, the confused thinking, the can't concentrate and so on.
You think to much because you are analyzing all of the what if's and trying to make choices about scenarios which havent taken place yet, or rerunning the ones that have. You are trying to get the best solution, which will protect you from danger.
The key word is danger. When trauma happened in childhood that hasn't been resolved, its an unconscious habit.
Think of it like as if you were a scared rabbit being chased by a hunter. You dart here and there, looking for any place safe to hide. Thoughts are the same way. You think this and then that and then the what if's, show up and you redo the same loop.

Answerers have given good advice. But before you spend some money on a doctor, try these web sites to know what the other symptoms are. I suggest printing them out and asking a couple people that know you well if they can find symptoms that describe you. People with ADHD don't always self monitor well. It would be advisable to ask parents/family members and a friend
Your symptoms could be caused by a number of other things too. So before you run out and spend a lot of money on doctors, get informed. It could be stress or something simple that you may have the ability to rule out. (Like, repeated lack of sleep;)

A lot of people get misdiagnosed with ADD, like gg says check out what you are putting into your body. and if there ids nothing there then it could be time for the doctors. Some people react like that to certain foods as well. Sometimes too much sugar will do it. Again pop.Chocolate is also a stimulant. And there are some of us who just get like that sometimes. How active are you physically. sports or even just walking sometimes helps.

Possibly stress.... not necessarily ADD.
If you tend to have a lot on your mind or you find yourself worrying a lot, keep a notebook and a pen handy, and write down whats on your mind... for example a list of things that you need to do, put them in order of importance.
This really helps because your thoughts are on paper so you dont have to keep them running through your head.

Your constant inability to concentrate would definitely bring
ADD as a possibility.You should have it checked out.

I have ADD and your symptoms could be ADD...could also just be stress related or depression related. I would see a doctor. they have checklist that you can answer and it will tell you definately. They probably have a website with the questions on there..

Do you drink anything with caffeine in it? Coke? Coffee? Energy drinks?
I would look at that first. Then talk to your doc.
