in my opinion, religion. but nobody likes that answer, so i'd just have to guess... the Bubonic Plague, or possibly Ebola. Hiv/Aids or Cancer.

Cancer and aids are both good answers, but how do you calculate how many people have died from cancer in the last 200 years let alone the whole of history? And aids has only been in the big leagues since the late 70's early 80's. I would say The Black Death. Or Plague. It killed a full 1/4 of the world population. The dead were stacked on street corners like wood. Lets see cancer pull that off. And aids, it takes to long to kill for it to even think about going for the title. Aids and cancer might win out in the long run, but as of now the most destructive "disease" in history was the Black Plague.

It depends on how you determine how you interpret destructive? In terms of the most people killed, it would probably be a newer disease because there are a lot more people than ever lived before... probably cancer or AIDS. If you're referring to the highest % of people killed compared to living, I'm not sure, maybe leprosy. If you're referring to killed the quickest, still not sure, but Ebola would have to contend.
