I don't know the medical term for it, but the Bubonic plague. It is/was caused by the fleas carried from rats in Asia that made their way to Europe. It killed at least 1/3 of Europe's population, some argue up to 1/2. There are many ways to answer your question, however. While the Bubonic Plague kiled approixmately 1/3 of Europe, the Influenza Epidemic after WWI killed aproximately 20 million- much less than 1/3 of the population, but along the same lines as in how many. You would have to balance not only the number, but its effect on history itself. You can argue its what causes AID's- it has claimed many lives and is a plague in Africa right now. There are many destructive disesases throughout history- I myself would personally (And may be wrong, my knoweledge of this matter is not complete) say its the Bubonic Plague- it radically contributed to a change in European Socio-economics, and brought about the end of Feudalism and a transition into Modern Europe. Also, I believe it killed the most in terms of percentrage/proportion. There may be a case in which an equal or even higher number died- but was probably not 1/3 of an entire population (Outside of perhaps a very small and isolated population, such as 1690's colonial America.)

According to what is known from ancient times, the Bible gives us a clear answer that the most destructive disease is leprosy. It has been found in recent times to sometimes be cured by means of long- term antibiotic treatments. The modern name for it is known as Hansen's disease. More than five million people are currently infected by this disease worldwide, and about five thousand infected are located within the USA. Who really knows the number infected throughout history! According to the Bible the disease was a picture of sin and the only way to be cleansed was and or is by the grace of God.

In Europe, I believe the black plague (from rats) (about 25 M) but in the Americas, I think Cortez & the Spanish brought smallpox and wiped out a large portion of the native Americans (about 25 M / 74% of the Indian pop). Also, the pandemic flu in about 1918 killed millions ( 20 to 40 M est).
So sometimes the issue is the portion of the population killed but as populations increase, there can be larger death #s but a smaller % of the population.

In recorded history? The Bubonic Plague. It killed 2/3s the population in Europe. Of course, as far as medicine in the Middle Ages is concerned, Religion didn't help much either.

ignorance has led to the death and destruction of millions of innocent lives globally and will until the end of time- ignorance the societal disease is most destructive, and that ebola virus is pritty gnarly too, hemmorhaging like that is a gruesome way to go, however not as widespread as the black or bubonic plague was back in the day or leprosy

The Black Plague killed 1/3 of the people in Europe, 30 million. That is high lethality.
Ebola is stalking Africa, with 95% or more lethality.
Syphilis was very widespread.
Aids is worldwide.
