I believe you are confused. A Jehovah's Witnesses gets disfellowshipped. They do so by having fornication, adultrey, smoking, excessive drinking, drug use, violence, criminal behavior. If they repent and are forgiven by the elders, then they won't be disfellowshipped. However, it is announced to the whole congregation and that person is humiliated because of it. They will say, Brother Smith has been reproved.

I can answer that. I am currently disfellowshiped. It is important to note that this is not the samething as being excomunicated.
Disfellowship can happen when a Endowed member has broken a comandment such as the law of chasity. Or anything that has landed them in jail. It is a form of repremand which does not remove a persons membership. The restrictions are simple. You are not allowed to participate in any way which would place you in a position to be responcible for some ones worship. No public prayers, no sacrament,. and for a man no use of the Priesthood. You can not hold any callings. For me it was decided that it would be three years and then the church would take another look and decide if they could change there decision. It is hard and not somthing I would like to go through a second time. I look forward to recieving my full membership once again.
If you wish to have your name taken off the lists of the church it is alot easier than you might think. Simply write a letter to your local bishop asking to have your name removed. That is all but you have to put it in writting.

Richmond is correct. I might add that disfellowshipping someone is not done publicly. Usually very few people even know about it. I know of a man in my ward who was disfellowshipped for a while, and later excommunicated. The only reason I knew of this was because I was in a position to extend a calling or ask him to offer a prayer in church, and I was made aware of the fact so I could avoid asking him to do something that he was not allowed to do. My wife still thinks he is a member in full fellowship. I do not know why he was disfellowshipped and excommunicated; these things are kept in confidence with the bishop.

There are a few levels of church discipline in the mormon church namely:
Probation - time determined by a Bishop were some privildges to act in the church are suspended for a time
Disfellowship - more serious than probation, can't do much in the church but still remain a member (usually adultery, fornication for younger members, those whose knowledge isn't great of the church, or haven't gone through the LDS temple)
Excommunication - no longer a member of the church, loses all membership rights etc. Have to be re-baptized to become a member again (usually as a result of adultery if you have gone through the temple, anti-mormon behaviour that could result in causing others to leave the church etc)
Name removal - name can be removed from the records upon written request to a local bishop which as the same effect as excommunication. They are reluctant to do this because of the seriousness of the exercise.

Generally this happens when you do something that goes against the churches laws, such as polygamy. It's more or less the same thing as excommunication, except that members are told to actively shun the disfellowshipped person.

The JW's are the ones who use this term. The LDS church does not do this easily and you have to actively work to get your name of their rolls and they will fight you along the way.

If you are asking how to have your name removed from their records you need to put it in writing. My dad did this and had his name removed alltogether.
