My grandson will be staying with me while his parents are on vacation and I want to be able to have any medical treatment he may need while they are gone. He is only 16 months old. Thank you.

if you really need a form try a law library, but as long as you have in writing from the parent/guardian that you are taking care of the child and can approve any/all medical decisions while he is in your care you should be fine.

they can type a note and signed ' giving the authority to make medical decisions on their behalf.

you need what is called a temporary power of attorney which you can get from any legal library.
