I don't know of a form. If they agree to your terms, they can write that into the settlement agreement. The first answer said write 'PAID in FULL' on the memo line of the check. This has ZERO legal meaning. A creditor can legally cash a check with that notation and continue billing you for any unpaid balance.

First, if you went to court that means that the issue has been resolved- FINAL and Binding.

Now if you only filed a claim and the have not gone in front of a judge to get a verdict, then you can settle out of court. In that case what you want to do is get a written statement showing that the balance is paid in full with the $750 payment. Also, write on check memo line "Payment in Full of (put in the reference, account # or invoice #)."
There is no standard forms that I know of, but you can just have them give you an account ledger (has to be generated from their system, cannot be handwritten) showing Zero balance after the payment.
