Not necessarily. If you know the requirements, and it isn't mentioned it might not be necessary. It deffinitely should not need to be through your school. I never went through my school to do community service. I usually did around 500 hundred hours a year when I was in high school and college. I've never written down every single thing I've done, because I never did it for the purpose of getting scholarships. If you already know of a scholarship you want to apply for that does say it is a requirement, I would keep track though. And it wouldn't hurt to write down the info of your volunteer work, just so you will have a personal record. It also may be a good idea to write down a record of the organizations and contact names, especially if it is an organization you work with a lot. If they want proof, other then documentation, this will be a good source, and a lot of organizations keep track of your hours at the organization themselves. It also is always helpful to have references at these organizations for applications such as colleges and jobs.

Yes, it should definitely be documented either through the school (if completed at school) or by the organization you are volunteering at. However, it is an idea to ask the scholarship granting organization what proof do they require of the community service completition, so it will be in the right form.
