Most of the time it is just an expression that is used to get a girl to put out.
however.. some men do experiance discomfort after being sexaully arroused for a period of time and not being able to release.
Like when a girl and a guy do things for a few hours and then she says... "no" to any form of sex. they guy has all this built up fluid and no where for it to go.
it does go away after a period of time.. and he is uncomfortable but it isn't life threatening.
