My guess would be that the old jaw injury is compressing a cranial nerve (either with scar tissue or just because of how it healed) or you have a strained facial muscle. I'd check with a doctor....and I'd go to a D.O. for this, not an M.D.

Could be referred pain from the old injury. The Ibuprofen helps with pain, which then returns as it wears off. You probably have an infection, either in the old injury, or in a nearby tooth. Try pressing down on each tooth individually, when you pass out from the pain, it was the last tooth you touched.
It looks like it's antibiotics for you. Get it seen to quickly, as infections in the jaw, if it is that, can do some serious damage.

You may be suffering from sinusitis which will get worse if left untreated. Ibuprofen will only give short term relief. Try some sudafed, they are a decongestant and just might help you. Look down at your feet. If the pain gets worse when you do that, then it's definitely sinus related.

I agree with Lisa, but you may honestly want to try hydrating yourself first. I used to get debilitating migraines and now that I am doing a better job at staying hydrated I have found that my headaches and migraines occur less frequently and are less intense.
