It could be that she feels undeserving of you and expects at any moment that you're through with her, or unhappy. I know it sounds ridiculous but I'm speaking from experience. It may also be jealousy. Talk to her, but be smooth about it. Tell her you've realized you've really hurt her and want to know what you can do to try to fix things. If she really wants to make it work, she'll be touched by your approach and maybe even realize she needs to fix some things.
She may just think you don't notice and don't care. Sometimes we girls just need a little extra reassuarance.
But if she continues to be defensive and doesn't want to change, you really need to consider the relationship. Relationships are all about give and take, not you give and she takes, but a good mixture of both.

She probably thinks you are asking dumb questions or thinks you should know the answers yourself and gets annoyed.
