Not flossing. Brushing just removes food from the surface of your teeth. Unfortunately food gets trapped in between your teeth and underneath your gums. That trapped food is actually rotting and therefore the bad breath. Go to the dentist twice a year, brush daily and floss 3-5 times a week. There are products on the market that look like a toothbrush without the bristles. You snap a piece of floss between the prongs and you can floss in 2 minutes. Also, use Mouthwash.
A quick tip - because the alcohol content in mouthwash is strong, use mouthwash before you brush. Why? If you brush first, the pores on the surface of your gums and inside your mouth are irritated from brushing and the mouthwash now stings. If you use mouthwash first (which also kills bacteria) you're more likely to rinse your mouth a little longer!

It could be food stuck in the teeth, plaque that hasn't been properly removed (the tongue is a place a lot of people miss), strongly scented food, smoking, or even a physcial ailment- diabetes can change the breath, and other diseases can impact how the breath smells.
