Most guys are ignorant so just let him have his moments just like we women have our moments.

You shouldn't need a questionare to tell you if he is in love with you. You should feel it for yourself. He probably can't express it as much as you would hope. Don't rely on a questionare for your situation though, its not a smart move.

And it is that kind of thinking that kills a realtionship. Its natural to be afraid of pain, but if he says he loves you then just believe him. Even if he does not completey believe it yet, if you just continue to love him and stop questioning his love for you he will fall in love with you. It is hard for a man to admit to loving a girl, so...at least take that into account.

Maybe he is getting annoyed with you constantly pestering him about his feelings. Chill out. Don't beg him for attention. Spend some time with yourself. Give him some space. If he calls you worried about what is going on, tell him how you are feeling then. If he doesn't call, he is not worth your time.
