The books have it that Lucifer was one of the angels.
This angel thought he could do a better job then God. God kicked him out of heaven. Lucifer being who he is hates God and has from then on went against everything God stands for.
Heaven or Hell - Considering people are not supposed to no what is in there heart, I feel this fate has been predetermined.
Separation of the worlds? Maybe spiritual worlds and only if you believe?
Since the devil is Lucifer and has other names, and Jesus is Gods son this has been answered to the best of my ability up there :0)
This is my two cents worth and I am sure there are others who will answer this in a more in depth way.

Before the creation of earth and man Lucifer was the most beautiful angel in the heavenly realm, second only to God himself.
This made him prideful, boastful and arrogant before the Lord. He felt he was better than God and he started a rebellion in the kingdom of Heaven. He and his followers were promptly cast out never to return. So from that point on he has done and will do what he can to lead people away from God and his love and now is called Satan or the Devil. The other questions I think you already know the answers.

The Bible itself answers your question. Please read the prophecy about the King of tyre (an actual country) but which describes satan the devil at Ezekiel 28; 13 - 19 . It mentions his start, why he became haughty, and his unfavorable future by the hands of his Maker.
Jesus and the apostle Paul both acknowledged his power and influence in religion. At John 8; 44 , Jesus tells the pharisees that the devil is their father. At 2 Corinthians 11; 14, Pauls compares Satan to an angel of light and says his ministers will pretend to be also.

Well, the devil, Lucifer. He was one of the Lord's most prized angels, until he became too prideful. He began to question the power of Lord, thinking he too could be just as powerful. He then was sent out of heaven and hell became the home of evil and those who don't believe in the one true God. People go to heaven by asking Jesus to forgive them of their sins and by admitting that they're not perfect. Because, I mean, who is? Then you accept Jesus into to your life as your savior. Building a relationship with Christ and living for him is the most rewarding decision one can ever make. It's not the easiest road. But heaven, ever-lasting life, is guaranteed. Hell is for those who don't accept God and his Son, Jesus Christ, who was sent to this world to die for our sins. Hell is eternal suffering. eternal. it never ends. There's no escape from it. The devil is a liar, who uses temptation to make people think sin is okay, etc. when all he wants is to bring more people away from the grace of the Lord. So basically, the devil is just not a guy you want to mess with, but he exists and those who trust in the Lord have no reason to fear him since God's holiness always overcomes evil. :] Hope I've made things a little clearer. God Bless

His name was Lucifer and he was an angel, the highest angel in Heaven and he decide one day that he wanted to be God but it did not work. Hell was created for the devil and his demons, the angels that chose to follow him. Man goes to heaven by excepting Jesus and Lord and Savior and they go to hell because they reject Jesus. Satan tryed to stop the birth of Jesus and he is now trying every day to take men to hell with him, by telling them that Jesus is not real. The sad part is that so many believe him.

Read Isaiah 14:12-15 Satan was an angel in heaven. He thought he would take God's place. His pride got in his way. eventually he convinced a third of the angels to follow him. There was war in heaven and Satan was kicked out of heaven. Eventually God will rectify this situation once and for all. and he will send the devil and those who follow him to hell.

Same way that the god became a god.
You can find this info in Genesis.
Essence of religion (in practical terms) is accountability for ones actions in this world.
Separation of world is divided by nasal septum.
Devil is a nice guy, he was an intellectual that's how he got involved in religion, I think he was not on such good terms with Jesus (only a conjecture).

He was a top angel ,and because of free will, he chose to challenge God and covinced 1/3 of the angels to join him.He was cast to earth but still has access to heaven, until he tries it one more time. Then he's cast down for good and he's not happy. Finally Jesus throws him into the "Abyss" for 1000 yrs.He gets loose one more time, tries to rally the troops and Jesus casts him into Hell, the Lake of Fire forever. In the meantime he,and his "Fallen angels" are a pain in our butts.

The devil is the devil because his momma was a momma devil or a devilette as they say. People go to heaven or hell because we believe that, not because we can prove it. Jesus spake (not spoke) to the devil and got the devil the hell out of there.

what lost penny said. To add it is pride that causes all falls. rebellion is just one form pride takes. Pride is at the root of all sin. If a person you know has a problem just ask questions about their reasoning until you find the pride it is always there and understanding that intial thought process can solve the problem unless they do not understand that pride is wrong, and that is where the Devil is he believes pride is his and good, never mind that if all have pride the result is this we live in now, war is the ultimate expression of pride.
