Throbbing pain is usually a sign of a vascular problem, however can be a nerve problem. it's possible you have a blood clot in your legs especially if your older or have varicose veins it's important to have it checked out because if it is a clot it could throw off a smaller piece and end up in your lungs(most probable which is called a pulmonary embolism), heart or brain.

Gout affects the big toe mostly. Sounds like nerve damage. See a Doctor ASAP!

you should definately see a doctor a friend of mine had a similar problem which turned out to be a bone infection and he ultimately had to have his foot amputated. It could definately be much less serious but see a doctor about it especially if the pain is intense when it occurs.

It is possibly an interdigital or Morton's neuroma. This is a painful nerve lesion between the toes due to wearing shoes with a too tight toe box. Make sure your shoes are not crowding your feet.
