First, be sure you're putting enough fuel into your body when you work out. That means eating small complete meals every few hours. Every meal should have a whole grain, a protein and a fruit or vegetable. This will keep you from being hungry and also rev up your metabolism.
Second, you should do at least 1/2 hour of cardio a day, and also weight train. Vary what you do to surprise your body. For example,if you run every day, your body gets very efficient at running and will burn fewer calories doing so.

Well, three weeks is a short period of time to really tone up your body, but the good news is that you should see the most dramatic results the quickest (has been my experience). My work out works well, if you have some basic knowledge of the machines at the gym. First of all, do 15-30 minutes a day cardio (you can start at 15 and work up to 30) I recommend the treadmill, elliptical (A+!!) or the StairMaster. Make sure you set the level as intense as you can handle and then rotate abs, legs, and upper body (I just do almost all of the machines depending on the day). Regardless of what you do, remember to STAY ACTIVE! No matter what your doing, rollerblading, walking in the park, yoga, swimming- whatever. And make sure you avoid/limit fatty foods. Stick to fruits, veggies, and lean protein. A good way to do this is to stick solely to the perimeter of the grocery store when you go shopping- the stuff in the middle is the least fresh, loaded with preservatives and usually, fatty.

To tone and condition the body that quick, you will need intense exercise. Start with a simple, but fun cardio exercise to get your heart, lungs, and large muscles in shape. Add flexibility exercise (stretching) to tone the muscles. Start a weight lifting program of light weights with lots of repetitions. You may want to try a very effective program called Isolated Body Conditioning where the muscles are always warm during your workout. You do a cardio workout for a short period of time, leift weights, stretch, and then repeat the whole process. The body conditions faster when the muscles are warm throughout the workout.

That would be difficult because it takes between 2 and 4 weeks just to get into condition. The first month should be gradual (to reduce the risk of injury or other health condition). Also, get a complete physical before you begin. At the end of the conditioning period, depending on your regimen of exercise, you can then expect to see results in about a month.

You should have started about 5 weeks ago because I think given the shape you're in its going to take about 8 weeks.
Watch what you eat, stay away from carb's. Take vitamins, and do some strength training or running every other day. Working out every day is too much your body needs time to recover.

I would suggest 1 hour a day on the treadmill with an incline of at least 6 and a speed of 4.0 m.p.h. But remember, you can't hold on. This will give your legs, butt, stomach, and arms a good overall work out, and burn a lot of calories too.

You can't tone up in three weeks. It takes time, and doesn't happen overnight.
But I would suggest swimming above everything else.
