The answer above is correct,but the actual entrance is on 5th Avenue. It is right across the street from Rockafellar Center.

The Cathedral takes up the entire block and that's why the entrance can be on Fifth Avenue but the official address, which is the archdiocese's offices, is Madison. Madison is the next avenue east of Fifth.
