Someone threatened to kill me. We haven't spoken since. They never asked for forgiveness. As a Christian, I know forgiveness is a must for going to heaven. God fogives us, we must forgive others. But God only forgives those that accepts Jesus's payment and asks for forgiveness. If someone doesn't ask or never talks to you or asks, must you forgive them?

You only have to forgive them if you want God to forgive you. If you don't care if God forgives you then you can have all the bitterness in the world you want against them.
Matthew 6:12 `And forgive us our debts, as we also have forgiven our debtors. 14 "For if you forgive others for their transgressions, your heavenly Father will also forgive you. 15 "But if you do not forgive others, then your Father will not forgive your transgressions.
BTW If he threatened to kill you, is it possible that you may have done some thing that hurt or offended him. Maybe you need to ask forgiveness from him.
Matthew 5:23 "Therefore if you are presenting your offering at the altar, and there remember that your brother has something against you, 24 leave your offering there before the altar and go; first be reconciled to your brother, and then come and present your offering.

Christians are not required to forgive those who practice malicious, willful sin with no repentance. Such become God’s enemies. No Christian who has been a victim of extremely unjust, detestable, or heinous treatment should feel forced to forgive, or pardon, a wrongdoer who is not repentant.—Psalm 139:21, 22.

At this point, forgiving him is more about putting it out of your mind and heart.Until you forgive him, he still has control over you. It still bothers you, right? Ask for forgiveness for both of you and pray for your enemies. This will give you your power back as well.

From what I learned in Christianity for 28 yrs. YES, you have to forgive in your heart. You don't have to talk to him/her or send a card saying you forgive them, but within yourself you must make amends.

You can ask the Lord to forgive them,,sometimes it is hard to come up to that type which you say,but if giving the chance I would say yes..by holding on to unforgivness,it isn't hurting the one who told you or hurt you,,it eats at your heart and you become bitter towards people,,so you are the one getting hurt because you are not free from them..
