I also suffer from the same kind of thing...waking up and not being able to tell if you are still dreaming or not is the worst part for me.
It really is almost impossible for anyone to tell you here what could be causing this, because every mind is different and deals with things in different ways. Also, dreams are "written" by the sub-conscious, so the images and situations in your dreams mean deeply personal things.
You mention a stress you have been dealing with for a long time, and the fact that the dreams are getting worse...in my opinion it is probably time to speak with a Mental Health Professional...that's not to say your "crazy" or anything, but talking with someone could help with the stress. Eventually, if the stress becomes too bad it won't be able to be relieved through dreams anymore and will begin to manifest itself in your day-to-day life.

Sounds like me :|
Instead of ignoring the stress and letting it build up and compress, you need to find a way to release the stress you have now, and then find a way to divert or deal with incoming problems.
I'm not sure what could be causing this, but it's probably not an individual event, if you've been having it your whole life.
Listen to piano instrumentals. Mix songs. Make playlists.
Just sit and listen.

You may be possessed by a demon.
Exorcist anyone?
Just kidding, you probably do not sleep sound enough to not dream and you are having nightmares instead. Tell the doctor and maybe some sleeping pills will help you. Some people's brain will not shut down enough for them to go into REM sleep and that may be your problem.
And quit watching scary movies !!!!!!!!!!!!!!!!!!!

I have suffered nightmares of mass destruction and death all my life. It wasn't until I was put on medication that suppresses over activity of the imagdyla (basically Im bipolar) that has stopped these. Might want to discuss this with a doctor.
