I'm no back "specialist" but as an athlete myself i've had a lot of back problems and am well educated from my chiropractor, atleast about the problems ive had. It sounds more like you have strained or pulled a muscle though than anything to do with your bones. Ive had pains where it feels like i'm being stabbed when i breathe in deeply too- it was caused by a rotated rib, so maybe you should get checked out for that, or atleast just go to a chiroporactor to see what he/she says, because even if it's muscle related they will be able to tell you the problem so you can get it fixed. If it is bone related, or nerve related, the only way to fix it properly is to get it adjusted by a professional. If it's muscle related it will eventually feel better so long as you dont work it until it feels better. Make sure your drinking enough liquid and eat more protein to repair your muscles faster.

Can't tell you how sorry I am to read your story. I hope I don't bore you but I also suffered for many years with back pain. It finally reduced me to being just short of needing a wheel chair. I was working in Bahrain at the time and was advised to see a specialist residing there. I know you had an MRI scan and I am sorry it did not give any evidence of a problem. I was a little luckier and my scan picked up a prolapsed disc which was putting pressure on a nerve in my lower back. Next day I had a none intrusive micro surgery operation and in 24hrs I was out of hospital with only a slight pain. Using the specialists exercises and walking a mile everyday for a couple of weeks I was soon back to normal. I really do care now what I lift and how I bend down but a small price to pay for being back to normal. I'd like to say that the specialist was from Pakistan and was made a sheik by the Bahrain Government for his services to the people of the region. Best wishes to you Malcolm.

I am a Neuromuscular Therapist and Certified Personal Trainer.
Your problem is most likely muscular in nature, or, more accurately, due to muscular imbalances. A very common cause of back and shoulder pain is actually caused by hypertonic (overly tight) muscles in the chest. Since you weight train is it possible that you are training your pecs/chest muscles more than your back muscles? Look in a mirror or have a friend check your shoulder position. Is one shoulder higher than the other? Is one shoulder closer to the mirror or anterior than the other? Viewed from the side, are your ears centered directly over your shoulder joint or is your head/neck noticably projected forward of your shoulder? If you have one or all of these postural distortions then you have muscular imbalances that are, to one degree or another, causing your back pain.
Another likely cause is that you have very tight muscles in your upper back or deltoid/rotator cuff. Check if the side with the pain also has limited range of motion compared to the other. If you have difficulty or pain lifting your arm past horizontal then your scapula is being restricted by overly tight muscles in your upper back and sending strain to other areas.
The simplest solution would be to find a qualified person to help you such as a Neuromuscular Therapist, Rolfer or deep tissue corrective massage therapist. They'll be able to help you identify muscular hypertonicity/imbalances, what's causing them, and correct the problem with deep massage and mild stretching. They can also teach you how to stretch or strengthen systematically to reduce or eliminate the cause of the pain.
Keep in mind that a chiropractor adjusts bones, not muscles. I would address the muscular issues first, the bones will take care of themselves once the muscles that are levering on them out of position are released. Also, a previous comment stating that acupuncture is the ONLY way to fix back pain is completely laughable. Acupuncture is good and has its place but does little to correct large or ingrained muscular issues. In my own practice I find that most pain is quickly resolved by simply improving blood flow, eliminating the spasm and restoring proper range of motion with deep massage and stretching. Tight muscles are painful muscles; release the tension and they have no more reason to hurt.
