Possibly all that you mentioned.

Some tips....

Read or listen to soft music.

Sleep in a dark room, or use an eye mask.

Use a white noise machine to drown out external sounds.

Try to have a light snack, like chamomile tea or warm milk 30-60 minutes befor you go to bed. Eating things like peanut butter or turkey also tend to make me a little sleepy too.

Avoid caffeine, and strenous exercise before bed. It can take hours for caffeine to be excreted out the body. (I're read to avoid caffeinated beverages after 10-11 am).

If you think food, drink and stress are the source of the problem then you might be right. Let me ask you a question, what's in your mind every time you experience trouble when you try to sleep? I may bet the answer is many, many thing. Because your mind is very strong, very very strong and full of energy. In fact many healthy people normally have a very strong mind, like yours. It just unfortunate it strong but wild or untamed. Now you get what I am trying to tell you?. OK, this strong but wild mind is driven by the daily experience or activities you have everyday. You may have solved many problems or challenges, but among them one ore maybe two may have been missed or be off your solutions and it stays remaining in your mind and your intelligent sense keep forcing you to deal with it. In common language a psychiatric term often call it "stress". Many will suggest you simply to take a stress pill or consult a doctor. Simply taking a pill may probably not a wise alternative, consulting a doctor may have a better sense, but it cost you time and of course money. The lengthy writing above has at least given you an idea about why this happened. Right, taming your mind. Taming mind means... meditation. Do meditation excercise! It cost you not even e peny! Every night conserve half or one hour before sleeping, make yourself relax, place a cushion on the floor (you can do it beside your bed in your bed room). Make sure it silent. Turn off the light as you don't need it. Sit down on the cushion, fold both your leg, place both your hand stacking each other. Start your meditation by closing your eyes, make yourself in a happy mood with smiling face, focus your attention in one subject. The best subject you may choose is your breathing. Inhale, and pay attention you are inhaling, exhale, pay attention you are exhaling. Repeat this again, again and again. You can stay on this course as long as you want, ten minutes, twenty minutes or even an hour! the longer the better. Please keep in mind, you must stay focus on one object. Every time you think about someting else, forget it immediately and get back to your focus i.e. your breath. Remember, every time! You will be amazed that this may happened many time and you have to always get back to your focus and also don't forget to keep relax. If you can maintain your focus for a long time without interuption then keep going as long as possible. When your attention is focused and uninterupted you are centered. Your mind will be calmed, a tremenduous happy feeling will surounds you. Get to bed, enjoy the relaxation and fall asleep. Doing it every night before bed. You can actually do it any time if you feel you are going to enjoy it, there is nothing wrong with it. It 100% harmless and costless!!! Good luck!

It can be triggered by food/drinks & stress your right, but you need to monitor your lifestyle so that you can figure out which one it is or if it is a combination of all three. Foods to avoid are the sugary ones & anything with caffine. Foods like bananas that are high in protein are great but not before bedtime as they are high energy foods. Slow releasing GI foods are best avoided.
In the meantime, all I suggest is that you relax, have you tried meditation? Reading can make you tired also, find a boring book though, like accounting or something, always works for me!
A friend of mine was a shift worker & had the hardest time falling asleep so he bought himself a relaxing machine. I dont know what it is called but it is similar to a water fountain only there is no water! There are different settings like birds or water or astorm, whatever works for you!
Best of luck :o)

Yo should actually go and see a doctor about this problem. It could be something medically wrong, or it could be a number of other things. Stress or depression will cause a person to not sleep, and if you are drinking beverages with caffine in them like coffee, tea, pop (soda) etc. that could also be your problem.

No one ever died from lack of sleep, a doctor told me that. If you stay awake, after a few days you will sleep. What everyone is really saying is they cannot live in their heads 24/7 and need escape. Sleep is the natural form of escape, but if you ever take sleeping aids. even weak ones, natural sleep is hard. My advice is just stick it out, walk, decorate anything, get yourself worn out to the point where all there is to do is sleep.

Generic Benadryl. Same chemical as sleeping pills, but 100 for $3 at WAL MART. You could also try Brain Wave Generators. Look it up online. I've found one that puts me right out.
