2/3 of the House and Senate must vote for it. Then 2/3 of the States. (the same for adopting an amendment). Each state has different standards for adoption, some have super majority (2/3) others simple majority.

Congress has the right to make or repeal amendments even without the vote of the people as provided for in the constitution.
