The cooking salt, widely known as common salt consisting of 1 part of Sodium (Na) and 1 part of Chloride(Cl); thus making the chemical formula NaCl. Although you could artificially make salt, but the cooking salt comes from sea water. Depending on the company making it, some harvest sea water and extract salt by sun drying the water in salt lakes. The concentrated white residue is then taken to salt factories where they are refined and sealed. Free flow salts undergo a special process of granulisation and adding anti-caking agents like baking soda. This prevents the affect of moisture and prevents causing lumps in salt.
If you are using low sodium salt for cooking, especially if a doc tells you to use if you are a patient of high blood pressure. These salts contains more of Potassium (K) then sodium. KCl is the formula for it.

Some of the salt we eat may come from the ocean-- but not all of it-- because we have salt mines in Arizona-- which they dig the salt, and clean it for table use.

Some does and it will be labeled as sea salt or bay salt.
most of what ends up on the table is mined from underground and processed for purity.
See this site for an explantion of how salt is brought to market.
