I don't know about ability but I had to write paragraphs in 5th grade and that was in the late 70's.

We were taught it's like a sandwich.
The teacher drew it out on the board.

Top layer of bread is the intro sentence introducing the topic.
Then at least two filling sentences discussing the topic.
Then the last layer of bread is the conclusion summing up the filling.

Before we actually wrote our own we had to break a pre-written paragraph in to sandwich layers. Rewriting each sentence in side a drawing of its layer.

if there in 5th grade and dont already understand how to write a paragraph then the previous years teachers have really failed them

That's pretty simple....make them READ READ READ READ READ and write what they're read about. Make them edit eachother's papers and point out where the subject changes. They should have known how to write a paragraph in the 2nd grade....

Ask other teacher who have been doing this for many years in the school you are teaching.

There are grammar text books that tell you this. Also, you could give them examples of paragraphs from the newspaper or... and help them analyze them, then write their own.
