I honestly don't know how children will be in heaven. Honestly, while it's assumed they will be in heaven, the Bible doesn't actually say what happens to children when they die (unless they are saved).
I would imagine we will all have a spiritual body at that point, but I don't know.

Interesting question:
I've read a few books and seen some tv shows about life and death / heaven and hell / the after life etc.
One book claimed that when abortion occurs / God sends an angel to catch the spirit and bring it to heaven where god then makes the spirit into a full grown perfect person..I thought that was pretty cool. Not sure about a young child..sorry
Then a television show I saw one time.....an older person said that as they walked towards the light they passed through like this spetacular ( i don't know how to describe..my impression was that it would be something like walking through a thin water fall - but not water if it makes any sense) and when they went through all there wrinkles were gone and they were young again - guessin maybe they looked as they did when they were 20-25.
All in all I don't think its possible to describe in words how good God is too people who believe in him in the after life

I think we will look like the way we perceive ourselves... This means, a child will look like a child or may be like Batman or Spiderman :) and an old man may look like he was used to be in his prime... We all do have a self image of ourselves in mind. And I believe we will look like that.
By the way, I am a Deist. I believe in God (not as described in 3 major books though) and I believe in a kind of afterlife. (But there will be of course no white fluffy clouds and big golden gates and a golden circle on top of our heads with a pair of white wings...)

You asked three separate questions, so I will answer each of them separately to the best of my knowledge from the Islamic point of view.
Note: I am assuming that all 3 questions are directed to what will happen to the believer of the religion- the believer in general, the believing child, and the believing old man
In response to the first question (1. How will every believer in God see oneself after death?), whether the man is a believer or not, the Angel of Death takes his soul and separates it from his body. This phenomena is mentioned in the qur'an as well as in a hadith (i.e. narration of Muhammad). The following incidents occur with the believer...
According to ahadith (pl.), "The Angel of Death comes to the [dying] believer, sits at his head and says, 'O you good soul, come out and receive your Lord's forgiveness and pleasure.' Then the soul flows out effortlessly just as water flows from the mouth of a waterskin.
Abu Hurairah (radhiAllaahu 'anhu) narrated that the messenger of Allaah (sallallaahu 'alaihi wa sallam) said: When the soul of a believer comes out [of its body], two angels receive it and rise with it towards the heavens, whereupon the inhabitants of the heavens say, "A good soul has come from the earth. Allaah has blessed you and the body which you used to occupy." [Muslim]
The rest of the details between this event and the Day of Resurrection are too lengthy to mention here and do not address your question. Summarizing quickly, the soul is brought up to Paradise, where he is greeted by Angels until he is brought up to the highest level of paradise unto his Lord to have his deeds 'registered'. Then he is sent back to where he came from (his grave) and stays there in the company of angels, until the day of resurrection and his grave is made expansive where he experiences bliss until the day of ressurection.
There is no doubt that the joy of the believers will be incomparable when they are led, group by honourable group, to the blessed Gardens of Paradise. When they reach Paradise its gates will be opened for them, and they will be welcomed by noble angels congratulating them on their safe arrival after suffering so much hardship and witnessing so much horror:
"And those who feared their Rabb (Lord; Self-Sustainer) will be led to the garden in crowds, until, behold, they arrive there; its gates will be opened and its keepers will say "Peace be upon you! Well have you done! Enter here, to dwell therein".
(39:73- Qur'an)
As far as what the believers will physically be, Abu Hurayrah reported that Allah’s Apostle (peace be upon him) said: "He who gets into Paradise [will be made to enjoy such an everlasting] bliss that he will neither become destitute nor will his clothes wear out nor will his youthfulness decline." [Muslim]
and
Abu Sa`eed al-Khudree and Abu Hurayrah both narrated that Allah’s Messenger (peace be upon him) said: "There will be an announcer [in Paradise] who will make this announcement, ‘Verily, there is in store for you [everlasting] health. You will never fall ill, and you will live [forever] and never die. You will remain young and never grow old. You will always live in affluent circumstances and never become destitute.’ [This is explained in] the words of Allah, the Exalted and Glorious, ‘And it will be announced to them, ‘This is the Paradise you have been made to inherit for what you used to do.’’[7:43- Qur'an]" [Muslim]
In response to the second question, (2. If a child dies, what will be his form in heaven after?), when the believing child dies, under the assumption a child is one has no sense of direction (of right and wrong),
Imaam Ahmed, Ibn Hibbaan and al-Haakim report from Abu Hurayrah that the Prophet (saw) said, "The children of the believers are in Paradise, being cared for by Ibraahim (known as Abraham in Christianity and Judiasm) (PBUH) [and in the narration of Abu Nu'aym, ad-Dawylami, and Ibn Asakir it adds- "and Sarah, until they give them back to their parents on the day of Resurrection"].
But after Ressurection, the children will also have the same fate as mentioned in the hadith above- youthfulness
In response to the third question (3. If a very old man dies, will he be the same old guy there?), they will also have the same fate as the child and any other believers.
