X rays didn't show anything, abdominal surgery showed only part of intestine was blue, when the vet moved it it turned pink, didn't find anything else. Considering costly MRI or CT. He seems depressed, lacks motor skills, stumbling, bad breath, fever, sensitive around neck and head. Came on over night. No vomitting or diarrhea. 12weeks old

Has he had a bad fall, or been hit by something ?

You have a 12 week old puppy but you didn't mention breed.?
I am assuming that prior to an exploratory surgery your vet. did routine bloodwork. Was this all normal?
What type of xrays? Thoracic, abdominal? How about history of vomiting or diarrhea? food changes, possible foreign body ingestion. Vaccine history, parasitology was done?
Seems as though you've left a lot unanswered. When in doubt try a second opinion either a different vet in the same practice or a different clinic all together. This is not normal for a healthy 12 week old puppy.

Since he's been to a vet, I assume he was checked for Parvo and Distemper. If he's a small breed, could he have been kicked or dropped, say by a child? Small breeds damage very easily, especially when young. Could he have eaten something poisonous in or around the house? I'm sending you my mental "blessing" of health. Can't think of a better way to say that.

Change his food from hard to soft and try Iam's. Check his water or look to see if he has ingested anything toxic while outside. Also, look for any bite marks such as a spider bite, scorpion sting or snake bite. Neighbors lost a Great Dane to a rattlesnake bite because it protected the kids from it. If it is an expensive puppy get the MRI.

it sounds like this puppy has possibly water on the brain. i lost a puppy years ago to this he was about this old. this is not your fault however it probably was born like this. it will run its course un less you decide to have him put down.

I THINK YOU NEED TO GO TO THE VET, GET HIM CHECKED FOR PARVO, AND MAKE SURE HE HASN'T GOT AHOLD OF POISON
