Or should each wife have equal say and only the husband can be in charge?

You shouldn't give any of the wives any authority. They might start getting some uppity notions like thinking for themselves. You want your women kept in their place.

Generally speaking the first wife is also the head wife in a multi-wife family...the wives that the husband marries later are referred to as sister-wives.
Polygamy is very much alive today in most parts of the world. And for those of the Christian religious leaning, there is more about multiple wives than monogamous. No where in the Bible does it say what kind of marriage is right or even if one should marry. If you read the parable of the 10 virgins, you will see that the bridegroom (singular) went into the wedding feast with the 5 brides. The bridegroom in this case is Jesus Himself...with 5 brides. Each marriage is individual and unto itself, however it is one family. To me this means that Jesus approved of polygamy and that it is the accepted way.

Today there are also polyandry families, one wife with more than one husband.

If gay marriages are now acceptable, then multi marriages should be as well...even more so.

Oh and don't forget ole King Solomon....he had 300 wives and 400 concubines....

I guess I got on my soap box...but I did answer your question 1st wife and sister-wives....

I'd think there would have to be a head wife. Not necessarily to be decided by age or tenure though - eventually I think it would become logical to the husband which one would be most suitable and that wife should be appointed as being in charge.

the head wife is already in charge when he is alive
the last wife is in bed and the seccond and 3rd wives are in the kitchen ,the 4th wife is doing the washing,

I look at it this way, if he had just one wife, he wouldn't have to worry about who is in charge when he is gone. By the way, even if he was home, she'd still be in charge ;)
