Some Elementary Schools have K-6 (mine did).
Most of the schools here go from K-8. (Here being the major city nearest to me in the US)
Some Middle Schools go 5-8 or 6-8.
Some schools go from grades 3-6 or 4-6...these are usually called intermediate schools
Our local high school goes from 6-12 but that is something very odd and due to being in a small town.

It does depend on the school district. It mostly just depends on how much room they have in all the schools. Generally, elementary school goes up to 5th grade and then 6th-8th grade are in middle school (we usually say middle school, not intermediate). But sometimes they keep 6th grade in elementary and only grades 7 and 8 are in middle school. It's like that in my school district, because we have seven elementary schools but only two middle schools. The middle schools would definitely be overcrowded with three grades, so that's why only 7th and 8th graders go, and then there's plenty of room for 6th graders in all the different elementary schools.

I live in Pennsylvania. K - 6 is considered elementary, 7 - 8 is considered middle school, and 9 - 12 is considered high school. But, my friend in the town next to mine has a school where all grades 3 - 8 are in the middle school. It really depends where you live.

Sixth grade is usually in middle school. Where I live, there is an elementary school that has 6th graders. Any school district can choose to have the schools set up like that. It is by decision.

6th grade is always middle school, the only time you'll see it in an elementary school is if it is a school that hosts kindergarten through 8th grade, but even then, those who have reached 6th grade are considered junior high students. In fact in the local schools here, the elementary school only goes up to 4th grade, before they start middle school in 5th grade.
